FAREWELL

It’s with mixed emotions that I inform you of my departure from the role of Wellness & Reablement Consultant for Loddon Mallee.
After 10 years in this role, I have decided to leap into the world of primary health, taking up a position with Murray Primary Health Network.

It’s been a pleasure working with staff across the Loddon and Mallee regions, I have learnt so much from your wealth of experience and expertise. The sector is fortunate to have passionate people such as yourselves, working hard to support older people in the community to be happy, healthy and connected.
I know you will continue to be really well supported by the great team of Virginia, Rosemary and Vicki until the vacancy is filled.
The two quotes above sum up my experience working in this sector - I've enjoyed the journey, and the dance.

CHSP & SSD - PROGRAM UPDATES
2022 has kicked off with news of the new Support at Home Program due to commence in July 2023.

The new program will combine the Commonwealth Home Support Programme (CHSP), Home Care Packages (HCP) Program, Short-Term Restorative Care Programme (STRC), and residential respite referrals into a single Support at Home Program. Read the program Overview.

Further essential reading is the suite of fact sheets developed by the department to help providers of Assistance with Care and Housing, Sector Support and Development and Specialised Support Services understand and prepare for the extension and transition to the Support at Home Program. 
Of particular interest to the Sector Development Team is the SSD Fact Sheet that points to extension of the SSD funded roles across Loddon Mallee until June 30, 2023.

Where are you on the

journey?
In light of impending changes, it might be timely and useful to undertake a 'stock-take' of your organisation's work relating to embedding wellness and reablement.

As always, keeping a copy of the Aged Care Quality Standards on hand when reviewing and planning is ideal to align your work to requirements within the 8 Standards.
Suggestions below and related resources might be useful to highlight opportunities for further improvement, and guide where you might focus your effort and resources:


1. Review and plan - From an individual, work team, program area, department or organisation-wide perspective review what's working/what's not working and develop strategies and actions to achieve required changes

3. Capturing & sharing success stories & staff practice - How does your organisation capture evidence of client stories/success/outcomes? How are staff supported to share and reflect on their practice? What opportunities exist?

4. Client & Carer Engagement/Outcomes - How do you know if what you're doing is making a difference for your clients? What measures do you have in place? How do you capture evidence?
