We encourage you to read through the welcome kit we mailed to you to familiarize yourselves with our plan, benefits, provider directory, preventive services, member forms, access to our incentive programs, and more.  You can also visit our website to learn about this and all of the resources available to you and your family through your Together with CCHP insurance coverage.  Also on our website is a list of frequently asked questions (along with the answers, of course). 
 
Remember that your premium is due on the 1st of the month.  Invoices for the next month’s premium will be mailed to you during the second week of the month.  If you receive an advance premium tax credit (APTC) from the federal Marketplace and fall behind in your premium payments, you are entitled to a three month grace period to pay any outstanding premiums.  Coverage will be terminated after three months of non-payment.  If you do not receive an APTC, your premium must be paid in full every month to retain coverage.
Provider Directory
Our Find a Doc tool is updated to reflect any changes in our provider network. Use this tool to locate the individual providers and facilities that best suit your needs from the comfort of your home. You can locate this tool directly on our website through your desktop or phone. You can also contact our Customer Service team at 1-844-201-4672 for help finding a provider.
Member Portal
Our member portal, shown below, is a helpful tool to become familiar with.  The portal allows you access to print replacement ID cards, review medical claims, pharmacy claims, and manage your online payment needs – including signing up for automatic payment.  By registering on the member portal the primary subscriber will receive a $20 gift card.
Wellness Incentives up to $90!

Foodsmart is a great tool that we’ve recently added to our suite of member incentive programs.  This app helps you save money on groceries, plan budget friendly and healthy meals, and even find discounts on healthy foods.  Read here for more information on how to sign up and get started. Preventive vs. Covered Services
Together with CCHP follows the recommendations of the United States Preventive Service Task Force to determine which services are considered preventive.  Broadly, “preventive services” are those that are fully covered without any cost to the member.  These services are only considered preventive if the provider bills them as such.  “Covered services”, on the other hand, are simply any service for which your insurer will help to control the costs and for which a member cost-share may be applied.  Any member responsibility will be applied towards your deductible or coinsurance amounts until the annual maximum out-of-pocket limit is satisfied.  You can receive an annual preventive visit at any point during the calendar year – it does not have to be 12 months from the date of your last preventive visit.  Talk with your provider to ensure that your visit is truly preventive.  Sometimes, just discussing other concerns can cause the visit to no longer be considered preventive.  Please contact our Customer Service team at 844-201-4672 for more information, or read our Preventive Service Guide.
Protect yourself and your family

With the additional risk of COVID-19 in our community, it’s more important than ever to keep you and your family as safe as possible and avoid preventable hospital stays by getting a flu vaccination.
This year the influenza virus and COVID-19 will be circulating at the same time, making immunization against the flu especially important this year to:
 
Avoid becoming infected with both COVID-19 and influenza at the same time (two respiratory illnesses) Keep your body healthy to more effectively fight COVID-19 should you become infected Avoid possible misdiagnoses as the symptoms may overlap Reduce the number of individuals who will require isolation due to overlapping symptoms of influenza and COVID-19 Avoid overwhelming health systems with a second respiratory virus (PPE, hospital beds, medical staff resources, etc.)
 
The best way to protect your family from the flu is by getting the vaccination, which can reduce the severity of flu symptoms or prevent the flu all together.
