This spring brings a new class of targeted therapy with outstanding efficacy, balanced by a favorable safety profile in clinical trials for the treatment of patients with moderate to severe atopic dermatitis (eczema) who failed or could not tolerate previous systemic or biologic therapy. While we still need better understanding of long-term safety, durability, and treatment success of our patients with atopic dermatitis in the real world, the Janus kinase (JAK) inhibitors hold great promise for patients suffering from lifelong recalcitrant eczema.

Tinted Sunscreens Provide Effective Photoprotection From Visible Light

In addition to UV filters, tinted sunscreens contain iron oxides, and in some, pigmentary titanium dioxide. By scattering and reflecting visible light photons, these ingredients provide effective photoprotection from visible light, known to induce erythema (redness) and pigmentation (discoloration).

Key points to consider when selecting and applying tinted sunscreens:

  1. Choose a tinted broad spectrum sunscreen with SPF of at least 30 that contains iron oxides.
  2. Look for iron oxides under “inactive ingredients”. While pigmentary titanium dioxide is listed as an “active ingredient” on the label, iron oxides may be listed as “inactive ingredients.”
  3. Consider your skin tone when choosing the correct shade of tinted sunscreen.
  4. For optimal protection, sunscreen should be applied 15 to 30 minutes before sun exposure.
  5. Sunscreen should be applied as the last step in your skin care, before make-up (if desired).

 
Taking Daily Vitamin D, Fish Oil Supplements May Help Protect Older Adults From Developing Autoimmune Disorders, Research Suggests

CNN (1/27, LaMotte) reports, “Taking daily vitamin D and fish oil supplements may help protect older adults from developing autoimmune disorders such as rheumatoid arthritis, psoriasis, thyroid diseases and polymyalgia rheumatica,” investigators concluded. In the study, “people age 50 and older taking 2,000 IU (International Units) of vitamin D3 for over five years had a 22% lower relative rate of confirmed autoimmune diagnoses.” Additionally, the study revealed “a possible link between taking 1,000 milligrams of omega-3 fatty acid (fish oil) and a reduction in autoimmune disorders, but the association was not statistically significant until possible cases of autoimmune disease – not just confirmed cases – were factored into the analysis.” The findings were published online in The British Medical Journal.

I recommend taking 2,000 to 2,500 IU of vitamin D3 daily, especially during the not-so-sunny months in Seattle. – Dr Young

Biotin Beware: Interference With Laboratory Testing

Many physicians recommend biotin supplementation for nail and hair disorders. Biotin supplementation can interfere with laboratory testing for thyroid function, cardiac function (troponin), 25-OH vitamin D, beta-human chorionic gonadotropin, hepatitis or HIV serology.

I recommend discontinuation of biotin prior to lab testing for accuracy. – Dr Young

HOW TO CARE FOR DRY, CRACKED HEELS

It’s easy for the skin on your feet to become dry and cracked, especially in cold weather. Fortunately, there are steps you can take to treat dry, cracked heels at home and prevent them from coming back.
To care for dry, cracked heels, follow these tips from board-certified dermatologists.

  • Limit baths and showers to 5-10 minutes. Bathing for too long can dry out the skin, making dry, cracked heels worse. Be gentle when blotting your skin dry with a towel.
  • Use a gentle, fragrance-free cleanser. This will help your feet retain their natural oils.
  • Moisturize within 5 minutes of bathing. Look for a moisturizing cream that contains 10-25% urea, alpha hydroxy acid, or salicylic acid, and apply it to your heels immediately after bathing while your skin is still damp and whenever your heels feel dry to lock in moisture.
  • Before bed, apply plain petroleum jelly or lanolin ointment. Consider wearing socks at night to avoid getting grease on your bedding.
  • Protect your heels. During the day, apply a liquid bandage over the cracks in your heels to create a protective barrier, which can help reduce pain, speed up healing, and stop germs from entering your skin.
  • Wear the proper shoes. If you have dry, cracked heels, avoid open-heeled shoes, such as flip flops or slingbacks, shoes that are worn down, or shoes that don’t fit properly.

 
Sometimes, cracked heels are caused by a medical condition, such as diabetes, psoriasis, eczema or a fungal infection. If your dry, cracked heels are severe or do not improve after following these tips, please consult a board-certified dermatologist.
