A View from the (Vice) Chair... 

Hi Everyone, in Lucy's absence I am reporting this View from the Chair.
As most of you know the whole structure of PSNC  and LPCs is under review. Recommendations have been made by the RSG (Review Steering Group) These will be released w/c 25th April ahead of a contractor vote on the proposals from May 23.

The LPC strongly recommends that all contractors study these proposals so that they can see the full picture and understand the consequences for their pharmacies. The LPC will keep contractors upto date with developments and key dates. It is likely that there will be regional events and we advise attendance; it is vital that all contractors are aware of the recommendations and are able to vote from a position of full knowledge. To ensure that the vote is truly representative of community pharmacy as a whole, it is crucial that all contractors engage in this important chance to influence contract negotiations for the future.

At our LPC meeting last week, we welcomed Susan Thompson as a visitor; Sue is Director of Pharmacy at University Hospitals North Midlands (UHNM) and also leads the cross-sector Pharmacy Leadership Group in Staffordshire and Stoke-on-Trent. Sue higlighted the issues in the hospital pharmacy sector - particularly outlining plans for the future, including improved discharge processes and electronic prescribing. She is keen to manitain strong links with the LPC and community pharmacy at large. She also highlighted work force issues and staff changes of roles.

Those who know me will know that I have moved on from Well and no longer sit on the LPC as a CCA rep. I have therefore also stepped down from my role as Vice Chair and so this is my last view from the chair.  However, I have been asked to remain as treasurer so I will still be around.

As Martin mentioned, he has now stepped down as CCA member of our LPC and as Vice-Chair. Martin joined the LPC as Co-Operative Pharmacy member in April 2005, and as such was our longest serving current member, serving as Vice-Chair for the last 12 years, and also as Treasurer since 2019. Myself, Lucy and all of the LPC wish to sincerely thank Martin for his time and dedication to the LPC as a member over the last 17 years, and are really pleased that he will continue to stay on as our Treasurer.
Finally, we are pleased to announce that Kieran Eason was elected as our new Vice-Chair. Kieran is an Independent contractor, being proprietor of Eason's Pharmacy in Wilnecote,Tamworth.

As it seems the COVID restrictions remain lifted, it is with some excitement that we approach the beginning of the holiday season. Long awaited holidays for our patients and teams are in full swing as we approach the Easter break. The lead up to this Bank holiday already seems to be very busy as everyone collects medication to take with them on their holiday. This will be the first holiday for lots of us abroad for over 2 years, I hope we all enjoy a well deserved break after a very busy period!
COVID cases continue to be high, throughout our teams and families although the symptoms seem to be milder in the majority of cases. We will be adjusting to the challenges this still brings to the pharmacy environment: Pharmacist gaps meaning temporary closures and attracting and retaining great staff. The fourth vaccinations now being available to vulnerable patients continue to build immunity in those who really need it.
As the vaccination programme continues to be rolled out the opportunity for Pharmacies to continue to be involved in the delivery of these to our local communities. As Technician involvement is encouraged this will help them build on their roles within the pharmacy and support their career development.
The new financial year means reviewing the successes and challenges from Last year and building on the opportunities for services on offer to patients through our skilled pharmacy teams: GPCPCS Referrals, NMS ,DMS and the  New Hypertension service. I and my team are looking forward to spending more time supporting our patients with their health and lifestyle concerns.

Also, following a couple of queries from local contractors, we would like draw your attention to PSNC's recently updated Guidance on Temporary Closures, following the end of the emergency provisions on 31st March; in light of this, contractors may be advised to review their Business Continuity plans to minimise disruption for patients and all stakeholders should a temporary closure be necessary.
Pharmacy Representation Review - RSG Proposals to be published soon

A proposed way forward to ensure that the support and services offered by PSNC and the LPCs are fit for the future has been reached by the Review Steering Group (RSG). 

The RSG has been looking for ways to solve the issues identified by David Wright’s Independent Review into the support and representation offered by PSNC and the LPCs.

After a year of extensive listening, exploratory work, analysis, and discussion within the sector, proposals that have broad consensus are being finalised. 

The RSG is once again engaging with AIM, the CCA and the NPA, as well as with PSNC and LPCs, seeking broad support for these proposals. Following this engagement, the RSG will publish its final proposals: all contractors will be asked to consider these, and to take part in a contractor vote to confirm that they are happy for the changes to PSNC and the LPCs be taken forward. 

What happens next? 

The proposals will be published shortly after Easter. The RSG will make the proposals as easy to read and understand as possible, and is working to the following timetable:

W/C 25 April 2022: Proposals published

W/C 23 May 2022: Contractor voting period begins

Contractors will have four weeks before voting begins in May to reflect on the proposals and will be encouraged to participate in a series of contractor events. The events will give a chance for questions and to hear from those who have worked on the proposals. 

Full instructions on how to vote will be shared with contractors in due course.

The Proposals and Vote 

The work undertaken by the RSG has been complex, and has involved extensive listening and involvement of stakeholders, alongside exploratory work to understand the external environment, potential costs, benefits, feasibility, and acceptability of various options for the way ahead. 

The final proposals will set out a range of ways in which the RSG believes PSNC and the LPCs could work better together to ensure that contractors get the best possible representation. This will include setting a vision for community pharmacy, reducing duplication between LPCs and PSNC, and ensuring contractors get the same levels of support, wherever they are in the country. 

Contractors will be asked to vote either to accept, or to reject, the proposals in full. 

Repeat Prescription Ordering 
For many years, community pharmacy contractors have operated Repeat Prescription ordering systems on behalf of patients. In some areas across the Midlands these have been superceded by CCG-run POD (Prescription Ordering Direct) Services, however this has not been implemented across South Staffordshire; with many GP practices now saying they will not take telephoned requests or paper repeat slips they have been directing patients to use community pharmacy repeat prescription ordering services. As a result, we are seeing an increase in complaints from practices about inappropriate requests for repeat prescriptions, for example orders being placed too frequently or for items that have been discontinued.
Whilst our LPC continues to advocate the use of electronic Repeat Dispensing (eRD) as one way of reducing the administrtative burden of repeat ordering and increasing efficiency for all stakeholders, this has not been universally adopted and will take time to roll out effectively and safely. 
So, the LPC will be looking to work with the CCG Medicines Optimisation team, Primary Care Networks and practices to increase the number of patients reordering their repeat medication via the NHS App. 

The NHS App enables people to:

Patients can register and prove who they are so they can:

  • order their repeat prescriptions and view, set or change their nominated pharmacy, where they want their prescriptions to be sent
  • view their GP health record securely
  • manage their first hospital or clinic appointment with a specialist, when they are referred by their GP, through the NHS e-Referral Service (e-RS)
Other services may be available dependent on the system used by their GP practice too.
We'll look to agree some communications with Medicines Optimisation colleagues soon, but for the time being your LPC suggests signposting your patients to the NHS app, available via the Google Play store or the Apple App Store.

CPCS Referrals Continue to Grow
Referrals to Community Pharmacy continue to grow, with nearly 6000 referrals in Stoke and Staffordshire since the start of the service, and over 29,000 referrals in NHS Midlands.
The NHS want referral numbers to quadruple this year, and to help drive activity we are currently recruiting CPCS implementers within the LPC.
The NHS nationally has also awarded a contract to Primary Care Contracting to deliver training to practices, to help them understand the importance of using Community Pharmacists more.
And CPCS is now appearing in various forms in GP and PCN incentives and Service Level Agreements.
A main focus for the service is to reduce demands on Practice clinical time. Another is to improve the patient experience. Here are just 2 examples where Community Pharmacists helped a patient access the correct services promptly.

Example 1
·        Patient  Called the practice at 8.40am Thursday  (as soon as the mad 8am rush when most appointments are scarce)
·        She was referred to CPS for ankle pain (non urgent) had the pain for 3 weeks
·        14.26 same day Pharmacy report had come back in need of a Doppler ultra sound scan as no pulse located
·        14.51 – GP called patient to discuss – no response – message left for patient to call back
·        Monday patient called back spoke to ANP same day – Doppler and foot exam done at GP
 
Example 2
·        Patient rang at 8.20am Thursday referred to CPS – dry cough
·        Seen through CPCS and referred back to GP as urgent case – spoke to GP same day by 11.31am
·        Believed to be a side effect of Ramipril and therefore drug regime needed adjusting
·        Pharmacist gave instructions to the patient to help with the dry cough
·        GP changed drug to losartan

Extended Care Services Relaunched
The Extended Care Services were due to expire in May 2022. However there is a lot of work being done to update and extend them until March 2023, and the Service Agreements were shared with all current providers on 30th March 2022 - contact the LPC if you are currently providing Extended Care Care Tier 1 or Tier 2a services and did NOT receive that email.
For now, no new contractors are being accepted. However pharmacists who are working in pharmacies with an existing contract CAN join the service. The do this by reading the service agreement and the PGD then enrolling through PharmOutcomes.
There are a few major changes to the service. The letter, which you can download here, details all the changes but in summary:-
  • A 7 day follow must be undertaken for ALL patients whether medication has been supplied or not.
  • There has been an uplift in fees to recognise this
  • There is a revised module on PharmOutcomes
  • NOTE Re-enrolment in the new module is required. There is a 4 week grace period during which you can provide the service, even though you have not enrolled. After that period you will not be able to record service activity.
Please aim to sign up to the service promptly, and certainly by the end of April 2022 - the signed agreement must be returned to england.pharmacy-westmidlands@nhs.net - please note that if a signed agreement is not submitted prior to the end of April 2022 then your pharmacy will be removed from the list of participating pharmacies.
Sexual Health Services in Staffordshire
There is now a new service provider, Central Health Solutions Ltd, and a new PharmOutcomes module and SLA, along woth updated PGDs. You will need to read the SLA and PGDs and re-enrol on the PharmOutcomes module, which has changed slightly to reflect the revised requirements of Staffordshire County Council. Pharmacies providing the service will also have a Sexual Health Champion who will be the link person for sexual health queries by Central Health and by team members
Humankind Services
Every contractor received a letter from Humankind last week inviting them to participate in 2 new services: Hepatitis vaccinations and Take Home Naloxone. Please note that signed contracts need to be returned to Humankind by the end of the month - emailed to: contracts@humankindcharity.org.uk

FROM NHS ENGLAND & NHS IMPROVEMENT WEST MIDLANDS REGION PHARMACY TEAM
Following up our updated final lists for Easter/May and Jubilee Bank Holidays 2022 Pharmaceutical Provision across West Midlands, which we shared on 1st April 2022, we have been informed by the DoS team that they have found a lot of inconsistences with the opening hours on pharmacy DoS profiles.
 
Could we please request all pharmacies to update their DoS Profile to reflect the correct opening hours for the Easter Bank holiday period, matching their notification to NHS England. We are aware of few pharmacies who notified us after the final notifications that they will not be opening as per the final list.
 
NHS England & NHS Improvement (NHSE&I) wish to remind contractors that they are required to include in their DoS profile their opening hours on Bank Holidays, Christmas Day, Good Friday and Easter Sunday; this must be the same as the information provided on the pharmacy’s NHS UK website profile. Please note that making changes to DoS does not automatically adjust the opening hours of your NHS UK website pharmacy profile; this needs to be separately amended on the NHS UK website.

Controlled Drugs Update
The Controlled Drugs Local Intelligence Network (CD LIN) for North Midlands (which includes South Staffordshire) recently held it's spring meeting. As part of this, there was discussion on a number of key themes, including:
  • Patient/family Safety around Controlled Drugs: a report of an accidental overdose of Buprenorphine in a child was shared - see the full report HERE. Young children were playing doctors & nurses, an older child found a Buprenorphine patch and was going to put the plaster on a younger child, however when the second child didn't want that the first child applied to their own thigh. The patch was obtained from a grandparent's room within the family home; the child was hospitalised however has made a full recovery. KEY ACTION - ENSURE ALL PATIENTS/CARERS COLLECTING CDs ABOUT SAFE STORAGE, ESPECIALLY WHERE CHILDREN SHARE THE HOME.
  • Temporary closures - these cause particular problems where the closure impacts on substance misuse clients. ACTION - ensure your Business Continuity planning includes consideration of how substance misuse clients will be managed in the event of an unexpected closure, particularly in relation to accessing relevant prescriptions for Opoiod Subsistution Therapy (OST)
  • Incident reporting requirements - contractors are reminded that ALL CD incidents and concerns should be reported to the CD Accountable Officer for NHSE&I via the National CD Reporting tool at: https://www.cdreporting.co.uk/ Note that "Incidents" are classed as something that has actually happened; "Concerns" are about potential incidents. The biggest numbver of incidents concern administratiuon errors such as record keeping and unnacounted for losses.
  • The CD Reporting tool also acts as a resource hub for managing Controlled Drugs too, including the opportunity to request destruction of out-of-date CDs. 
  • There have also been a significant number LASA errors involving CDs, especially involving pregabalin and gabapentin
  • Security of returned schedule 3 and 4 CDs - related to possible illicit diversion of returned medicines from a community pharmacy - KEY ACTION - ENSURE PHARMACY SOPs FOR MANAGING PATIENT RETURNS ARE FOLLOWED

Staffordshire Together for Carers Service is the jointly commissioned carer wellbeing service offering support to carers of all ages in Staffordshire.  They have dedicated Carers Information and Support Officers who can offer support to carers aged 18+.  They also have dedicated Young Carer and Family Carer Practitioners who can offer support to young carers from the age of 5.  Staffordshire Together for Carers Service offers a range of one-to-one support for carers and a wealth of wrap around support. 
Working in partnership with Changes, Staffordshire Together for Carers Service can offer specialised emotional wellbeing support to carers via Changes well-established 10-point Wellness Action Plan, age-appropriate workshops and peer support groups.  Staffordshire Together for Carers Service is also able to offer, via its partnership with Citizens Advice, specialised support to help carers manage their finances and maximise their income with one-to- one support with an advisor.
Staffordshire Together for Carers Service wrap around support includes peer support groups, both face-to-face and virtually, contingency planning, access to a 24/7 Carers Help and Talk telephone support service and access to a digital carer community network.
