IMPORTANT UPDATES ARE BELOW!
Thank you for your support of CHWs in Minnesota. The Alliance is continuing to work hard to support the passing of state funding for CHWs. 
 
A bill in the Minnesota legislature (UPDATED: HF 4706 – starting line 56.30includes funding to support the research, promotion, and development of Community Health Worker (CHW) models in Minnesota.

Help support CHWs by reaching out to your House and Senate representatives to encourage them to support this funding. Click here to learn who your representative is. UPDATED: We especially encourage you to reach out if you are a part of District 2 in Minnesota (Becker, Beltrami, Clearwater, Hubbard, Lake of the Woods, Mahnomen, Otter Tail, Wadena counties). 
If an organization you are a part of would like to write a letter of support for CHW funding once complete, please send them to Alliance Executive Director Adri Lobitz at adrianna.lobitz@mnchwalliance.org.

UPDATED: Dual Training Grant Request Application

Pipeline Dual Training Grant has officially opened for application until April 12.

Community Health Workers are an occupation that is available that the grant could pay for up to $6,000.00 per employee for that education.

Alliance Leadership Session in Bemidji
The Alliance is hosting a leadership training in Bemidji, Minnesota on Thursday, May 19th, the evening before the start of the Rural CHW Conference. 

The night will start at 5:30 pm with 30 minutes of networking and socializing with snacks provided then lead into updates from the Alliance and an introduction to the workshop beginning at 6:00 pm.

Exploring and Fostering Leadership will focus on developing strong communication skills, discovering your leadership style, and diving into values-based leadership.

CHW Learning Circle Meeting

TODAY, Wednesday, April 5th from 3:00-4:30 pm the Alliance will host the monthly CHW Learning Circle! 

The introduction to health coaching will provide a brief description of ways to engage clients in healthy lifestyle changes. You will learn how to assess motivation to change and learn how to help clients set realistic goals that are achievable.
 

PRESENTER:

Renee Gust, M.A., R.N., Senior Health Promotion Specialist with Hennepin County Public Health, has been providing health coaching training to hundreds of medical staff for over nine years. She received health coaching curriculum training from the Center for Excellence in Primary Care, University of California-San Francisco. Renee has over 25 years of experience working in hospitals and clinics as a staff nurse.

National Public Health Week is an opportunity to recognize the contributions of public health and highlight issues that are important to improving our nation’s health. CHWs are an important workforce in Public Health that make a huge difference in the communities they work in. 

This year’s theme is Public Health is Where You Are — and during each day of National Public Health Week, we focus on a particular public health topic.
Here is the schedule of focus and resource for each topic:

Community Health Worker role with PrimeWest

This role is remote. 

This position focuses on improving the health status and social drivers of health of PrimeWest Health members, both individually and as a community. The Community Health Worker (CHW) is an essential part of the PrimeWest Health Care Management team and is responsible for primarily working in the community with members who benefit the most from their services. CHWs are trusted, knowledgeable frontline health personnel who typically come from the communities they serve. CHWs bridge cultural and linguistic barriers, assist with access to care, address social drivers of health, and improve health outcomes. As critical links between their communities and the Care Management team, CHWs work to reduce health inequities, boost cultural competence, and empower individuals and communities to achieve better health through advocacy, education, and support. CHWs play an integral part in reducing emergency room use and unplanned hospitalizations; increasing access to holistic, integrated health and dental care; and connecting members to covered services. The CHW works closely with medical and behavioral health providers, primary care teams, care coordinators, county partners, and other agencies to improve member care and outcomes. CHWs travel by automobile throughout their community in order to carry out some of their job responsibilities.

Community Health Worker- Clinic Case Management at Sanford Health

Bemidji, MN

Department Details

Clinic-based Community Health Worker position at Sanford Bemidji Medical Center. M-F days with office hours and opportunities to outreach to local shelters and community centers. No holidays or weekends required.

Job Summary

The Community Health Worker (CHW) will serve as a bridge between under-served communities and the health and social service systems. As a non-clinical role, work within the patient-centered medical home model, which embraces a team approach to chronic disease self-management. Provide information and assistance to help patients navigate a complex health and human services system. Strong advocacy and raising awareness of individual and community health needs are necessary for efficient and culturally sensitive work.

Must strive to build capacity for access to services and community engagement. Strong communication skills from a non-judgmental perspective will be necessary to engage patients in their own care, articulate clearly and effectively when advocating for patients and community groups, and assist members in understanding and effectively navigating complex health systems. Effective active listening skills are essential. Actively engage in relationship building and work from a team-based perspective.

Collaborate on goal setting and care planning within an inter-professional team context. Required to give presentations to community groups, and requires strong public speaking and presentation skills that include diplomacy and tact. Strong organization skills and the ability to anticipate future planning needs is required. Effective coordination skills are necessary in facilitating appointment attendance and individual plans of care. Serves as a point person for issues and related solutions while maintaining appropriate boundaries.

Hold knowledge and experience of ethnic and cultural populations, government partners, community stakeholders, and other organizations. Model effective problem solving skills to empower patients to take an active role in their health care. Must have a thorough understanding of self-efficacy theory and its application when working with others who have chronic disease. Embrace cultural sensitivity when seeking information and working with others.

Documents completion of assigned activities and support of community members as required. Provide services under supervision as directed by state or other regulatory requirements. Strictly adhere to the scope of their duties and will not give medical advice under any circumstance. Strong adherence to all patient privacy laws in health care settings and the community is essential.
