COVID-19 Community Grants

Dear Community partners, 

As you know, The Oprah Winfrey Charitable Foundation awarded $5 Million to support Live Healthy Chicago (LHC). LHC is a collaborative formed by Forty Acres Fresh Market; MAAFA Redemption Project; My Block, My Hood, My City (M3); Rush University Medical Center; and West Side United. LHC aims to dismantle the systemic racial and health inequities exacerbated due to COVID-19 in 20 majority Black and Brown target communities in Chicago by implementing, funding, and sustaining both immediate and long-term strategies. 

This two-year grant will provide $2.9 million in community COVID-19 Response and Recovery Grants to community organizations based in the city’s most impacted black and brown communities. The following 7 organizations will receive $50,000 to support their current COVID-19 response and recovery efforts:

  3. Enlace
  4. Greater Auburn Gresham Community Development Corporation
  5. Latinos Progresando
  6. Northwest Side Housing Center
  7. South Shore Works

COVID-19 Community Grant FAQ

What is the Live Healthy Chicago Community Grant and who is funding it?

Over the next 2 years, LHC seeks to distribute $2.9 million in community-based grants to organizations who are implementing COVID-19 response strategies in the following black and brown communities: 



The grant is funded by the Oprah Winfrey Charitable Foundation and will support projects that address COVID-19 through prevention, education and support services.

How large are the grants and who can apply?

501(c)(3) nonprofit organizations that serve one of the 20 target community areas are eligible for the grants. Organizations can request between $10,000 and $100,000 for projects. Grant funds must be used to address the impacts of COVID-19 and project grant funds need to be spent within one year (365 days) of the disbursement date.  Organizations can submit one application.

How will Live Healthy Chicago decide which applications to fund?

LHC will evaluate applications based on:

  • Importance of the organization to the surrounding community

  • Proposed use of the grant funds

  • Feasibility of the project based on organizational capacity and available funding

  • Strength of the overall application

 

LHC aims to fund strong applications across a number of target neighborhoods.

What are the reporting requirements for grantees?

Grantees will be required to provide an annual report documenting:

  • Project outcomes

  • Number of individuals impacted

What supports will grantees receive?

Grantees that are providing community wellness or health outreach programs will be able to leverage select training opportunities provided by LHC partner, Rush University Medical Center (Rush). Rush will also facilitate an LHC learning collaborative with regular webinars that provide an opportunity for CHWs and other community health outreach workers to sharpen their skills, share their experiences, seek consultation, and make peer connections. 

When are applications due and when will the grant awards be announced?

Applications are due by 11:59pm on August 31st and the grants will be announced by October 1.
