WHAT'S NEW
The COVID-19 Resources for Skilled Nursing Facilities webpage is frequently updated with links and information to assist you, including these most recent updates:

 ➢ The Society for Post-Acute and Long-Term Care Medicine (AMDA) has become the first national organization to formally support the adoption of a national POLST (Physician Orders for Life-Sustaining Treatment) form. Patients who have a POLST, who may be POLST appropriate but never engaged in advance care planning conversations, or who may be diagnosed with conditions that put them at high risk of dying from COVID-19 should be approached about their treatment wishes and POLST. National POLST encourages facilities to consider the following guidance for Completing POLST Forms in Crisis Standards of Care.

 ➢ MyDirectives helps people create, share, and update their medical treatment wishes, palliative and hospice care preferences, organ donation status, and other critical information on the device, and in the format that is most convenient for them. MyDirectives is secure, easy to understand, and always FREE to use.

 ➢ The Decision Guide is a free tool to help with residents, their families, friends, and caregivers to understand avoidable transfers and how they can be involved in the decision. It is useful in helping support staff discussions addressing resident and family insistence on a potentially preventable hospitalization.

 ➢ Register to learn more about Dementia Care in the Time of COVID-19, part of the Master Clinician Series presented by The Center to Advance Palliative Care (CAPC), on Friday, August 14th, at 2pm ET. During this webinar you will learn tips on what to do—and what not to do—for people living with dementia and family caregivers during the time of COVID-19.

 ➢ Enroll today in Basics of Health Equity: Relevance & Responsibility of Equity for Healthcare Leaders on Wednesday, August 12th, at 2pm ET. During this free webinar, cultural competency expert Emebong Martin, MPH, Regional Health System Director for the Alzheimer Association, will engage you and other healthcare leaders, including those on the front lines, in understanding health equity, how it operates in post-acute care settings, and opportunities for applying a health equity lens to the delivery of care.
