Weekly Newsletter: What if it’s not a yeast infection?

Most women have had a yeast infection at some point in their lives -- we treat it, and it should resolve quickly.

But what if you’re getting recurrent symptoms? Tube after tube of Monistat and still experiencing discomfort?
 
Maybe it’s not a yeast infection!
 
If you are having vulvar symptoms such as irritation, burning, dryness, itching, pain, or discharge, there may be something other than yeast going on. Symptoms could be from: 

  • Bacterial vaginosis or “BV”: an imbalance of the normal bacterial flora of the vagina and vulva that can cause irritation, itching, discharge, and/or odor. Over the counter medications will not treat BV, but it should resolve with antibiotics. However, BV can become recurrent in 20-30% of cases, in which case additional treatments are needed. 
  • Lichen sclerosis: a chronic inflammatory skin condition (similar to eczema or psoriasis) that affects the vulva, causing itching, burning, and even cracking of the skin. This is most common either after menopause or in girls before puberty. Chronic steroid ointment has been the standard of care for many years. There are now studies, however, that show that the Mona Lisa CO2 laser is effective at treating lichen sclerosis and can seriously cut down on steroid use. 
  • Genitourinary syndrome of menopause: 70% of women after menopause will develop this condition that causes thin, fragile, and painful skin around the opening and inside the vagina. It can even lead to bladder weakness and leakage! Treatment can be estrogen creams or the Mona Lisa CO2 laser.
     
  • Contact or allergy-mediated inflammation: the skin of the vulva is very sensitive! We frequently see women with itching, burning, and discomfort caused by fragrances and chemicals in soaps, lotions, and even pads. The rule of thumb is the simpler the better! These symptoms will resolve with stopping the offending product and a short course of steroid ointment.
