WHAT'S NEW
The COVID-19 Resources for Skilled Nursing Facilities webpage is frequently updated with links and information to assist you, including these most recent updates:

➢ JAMDA (The Society for Post Acute and Long Term Care) has a compilation of all COVID articles that include articles about SNF visitation and reopening guidance from other parts of the world such as Canada and the Netherlands. Also be sure to check out information on what individual SNFs are doing such as designating Essential Family Caregivers and ways to mitigate loneliness and isolation

➢ For additional resources about Nursing Home Reopening and Visitation, check out:
OPPORTUNITIES
There are also training resources available to help guide you through COVID-19. Check out these opportunities for patients and staff:

➢ The CMS Targeted COVID-19 Training for Frontline Nursing Home Staff & Management is available now, with five specific modules designed for frontline clinical staff and 10 designed for nursing home management. The training is available on the CMS Quality, Safety & Education Portal. See the instructions HERE on how to access this training.

➢ Alliant Quality, the Medicare Quality Innovation Network-Quality Improvement Organization (QIN-QIO) for our region, provides assistance to nursing homes to improve your infection control program with its Infection Control—Quality Improvement Initiative Affinity Group 30-minute Quickinars. This educational series will guide you on creating an action plan for improvement and identify specific steps to establish a strong infection control and surveillance program. The next Quickinar is Wednesday, September 2, at 2pm ET: Using FROGs for Good Hand Hygiene
REMINDERS
➢ Your feedback is needed! Duke Health is exploring the opportunity of e-prescribing controlled substances directly to long-term care pharmacies for patients discharged to SNFs. If you have not done so already, we are asking for your assistance by completing this one-minute survey.

➢ If you or your staff were unable able to attend last month’s HOPE SNF Collaborative Webinar on our SNF TeleHOPE program and Communication in the Age of PPE, click HERE to watch the recording. You can access materials from the webinar by clicking HERE.

➢ Mark your calendars! The next HOPE SNF Collaborative Webinar will be on Thursday, September 10th, from 8a-9:30a.
DUKE HEALTH UPDATES

Earlier Patient Discharges
Our hospitals are already seeing a rise in the census, and—in anticipation of flu season compounding this rise in the coming weeks—we are asking for your help with transitioning patients to your facilities earlier in the day. You may receive requests from case managers to accept patients in the morning if you can possibly accommodate the earlier discharges. Of course, we will continue to send patients later in the day as needed. Please don’t hesitate to reach out to Pat Kramer, DUHS Senior Director of Case Management, at patricia.kramer@duke.edu if you have questions or comments about this request.

SHOUT OUTS
Our Shout Out this week is to the staff at The Pavilion at Croasdaile Village for sharing their experiences about the participation in the SNF TeleHOPE program for improving transitions of care at last month’s HOPE SNF Collaborative Webinar. Below are some of their thoughts on the benefits of this program. Thank you, Croasdaile Village, for helping Duke Health care for our patients, their loved ones, and each other.
