Welcome to LPCN’s October bulletin. At LPCN we are continuing to work with our partners during this challenging time and thank everyone for their dedication to providing high quality services for people in Leeds.

In this bulletin we have an opportunity to have your say through a Whole Systems Approach to improving palliative and end of life care services in the community and the way they link with acute hospital services.

We would also like to recognise the amount of virtual education and training that has been delivered recently – congratulations to everyone involved. We hope you are keeping safe. We will continue to provide support and guidance wherever we can.

Your Chance to Improve Care for Dying People in Leeds

LPCN is holding a series of online events using the Whole Systems Approach to improve Palliative and End of Life Care in the Community in Leeds.

The main focus is on all community (out of hospital) services and how they interact with each other and hospitals to meet patients and their family’s needs. The sessions are 1.5 hours long and aim to develop a clearer and shared understanding of how the local system works.

We want to get your views of what works well in Leeds and also identify any areas of duplication or gaps. For event dates and more information, click here.
 

Education and Training Update from Trish Stockton, LPCN Education and Research Lead, Head of Learning and Teaching at St Gemma's Hospice

We have worked hard throughout the pandemic to continue to support end of life care education across the City and in the last three months we have delivered training to more people than we would normally deliver to in a year.

The key point for us around COVID-19 is that it continues to be an extremely difficult and challenging time for health care and professionals. We could clearly see that the learning need was there in the City and regionally and there was an opportunity for us to share the knowledge of our experts and specialists with others. Initially we modified our training programme to relate it specifically to COVID-19, which evolved rapidly as more guidance became available. We then focussed on key areas of learning and developed resources to support them.

Key City-wide collaborative training and education achievements:

  • Produced a video to support the LPCN guidelines on Symptom Management of patients with Covid-19 (on LPCN website – currently 270 views). 
  • Produced guidance and training pack on Advance Care Planning for the LPCN website and community staff. 
  • Produced 5 training videos for Leeds & York Partnership NHS Trust in how to care for patients at the end of life. 
  • Verification of Expected Adult Death for Leeds Nursing Homes via ECHO/ tele-education – St Gemma’s Education team worked with the Leeds Community Health End of Life Care Facilitators to provide training to 42 staff from nursing homes in Leeds.
  • Both hospices provided a comprehensive training programme on Advance Care Planning/ ReSPECT to GP practices with over 100 people attending sessions on ZOOM. 
  • LPCN website – an invaluable site for training and education resources over this time. 

Sharing Learning Far and Wide

Nursing staff from Cancer Relief Gibraltar recently took part our Verification of expected Adult Death Training, delivered by St Gemma’s Hospice Academic Unit of Palliative Care.

Their feedback was extremely positive, and we are delighted to be supporting others and sharing learning far and wide!

Supporting Local Colleagues to Develop Essential End of Life Care Skills

LPCN’s Education Group and St Gemma’s Hospice have worked collaboratively over the past twelve months to provide training and support to clinicians working for Leeds and York Partnerships Foundation Trust (LYPFT).

Alex Irving from LYPFT said: “Together, we have undertaken a yearlong project to support clinicians at The Mount (LYPFT) to obtain the skills necessary to provide end of life care. The services at the Mount are acute inpatient wards for people with dementia and those over 65 with acute mental health needs. The Mount had sought help from LPCN after an increase in people receiving end of life care on the wards, recognising the need to improve skills and knowledge to improve quality of care.
 
Sadly the unit saw an increase in people receiving EOLC as result of Covid-19, but we entered these uncertain times with confidence and competence following the work the previous year to increase knowledge and skills. As a result, staff at the Mount are proud of the quality of care delivered and feel able to share their learning as we continue to grow and develop our workforce. The Mount are very grateful to the ongoing support and amazing role models from Wheatfield’s Hospice and The Palliative Care Team at Leeds Community Healthcare NHS Trust. We are also now proud members of the LPCN.”

Message from LYPFT colleagues regarding ReSPECT

Please note that LYPFT have not yet started using the ReSPECT documentation and are continuing to share the DNACPR form. We will ensure all partners are kept up to date with LYPFT’s plans with regards to implementation. Thank you.
