ESTEEM Awards 2020

Hi Everyone,
As we approach the end of a tough 2020, we are sending out the last round of ESTEEM nominations that came in last month.   These were social prescribing folk who have been recognised for their care, kindness, perseverance, support, empathy and more, during the pandemic.

We are so grateful that people have taken the time to recognise their colleagues and share their inspirational stories.  This has been a difficult year and it has been inspiring to see how passionate people are about continuing their amazing  social prescribing work in these trying times.

The latest set of nominations are from Waltham Forest, Hertfordshire, Brighton and Hove, Hulme, Plymouth, London and Virginia USA.  To see their full stories and read about previous nominees visit the ESTEEM gallery.

At the beginning of November we launched the 2021 Social Prescribing Network Awards - These will be judged by panels for each award and the winners will be announced at the online event during our 3rd International Social Prescribing Network Conference. Click here to find out how you can nominate someone for a Social Prescribing Network award.

Thank you once again for taking the time to recognise people in our social prescribing family.  And finally a big thank you to Ali, Rachael and Joy at Chamberlain Dunn who came up with the idea and worked with us to manage the processes involved.

Marie

Co-Chair, Social Prescribing Network

Waltham Forest Social Prescribing Team

First off, three nominations from the London Borough of Waltham Forest.
Shamal Warraich, Social Prescriber Link Worker and the Waltham Forest Social Prescribing Team have looked after each other with, for example, a WhatsApp group for sharing tips, asking advice, encouraging each other and exchanging information about relevant services.  Steffie Vial is great for advice around homelessness and Shamal Warraich for adult social care queries. The WhatApp group is also a great source of banter and laughter.
The team joined the Primary Care Networks early in the lockdown, an extremely challenging time where they had to learn to adapt quickly by creating a virtual team space, working remotely while training. They offered practical and emotional support by telephone when patients experienced loss, both bereavement and unemployment. They learnt quickly what support local agencies were offering in terms of financial advice services, welfare, benefits, and income support.
The team personally requested GP shielding lists to reach the most high risk and vulnerable patients as quickly as possible to ensure they had access to food and medication. Patients are supported to access all relevant resources and services.  Positive feedback has come from Clinical Directors, GPs and patients alike. ‘In a trying time, we have held each other up and shone a light on how necessary social prescribing is, more than ever,‘ they say.

Herts Hospital and Community Navigator Service

Mary-Ann Lindsay, Area Manager West, Herts Hospital and Community Navigator Service has provided incredible support to ten new PCN Link Workers, as well as 15 existing community navigators.  She worked tirelessly to ensure clients got what they needed – covered weekend shifts, even delivering shopping herself when no other solution was possible. She organised outreach to thousands of patients with new link workers ringing shielded patients.

Brighton & Hove LGBT Switchboard

Vernon Collins, Trans and Non-Binary Link Worker, Brighton & Hove LGBT Switchboard joined this pioneering project during lockdown. It is a community facing a host of health inequalities worsened by COVID and Vernon has been doing  incredible work with people in all kinds of difficult situations - from sourcing  internet connections and tablets or housing and benefits, to ensuring people continue to access health appointments, food, or heating. Most importantly, Vernon ensures that trans and non-binary people have access to their community.

Growing For health Team, Sow the City

Jon Rose and the Growing For Health team, Sow the City, Cornbrook GP Surgery in Hulme created a new wellbeing garden funded by Age Friendly Manchester and supported by local social enterprise Sow the City. The garden provides opportunity for local people to learn new skills, take part in physical exercise, get access to healthy and free food and meet new people.  During lockdown, Sow the City provided webinars so people could keep growing at home. Prescribe a Plant was developed as a simple way to keep up the growing activity in a safe way.

Team Breathe, Breathe Arts Health Research Team

Yvonne Farquharson and Team Breathe, Breathe Arts Health Research Team in London, have responded to the challenges quickly, creatively and imaginatively. Alongside reimagining all the existing arts and health programmes for COVID-safe delivery, they have developed an NHS staff wellbeing package, responding to needs identified by NHS partners. Their close relationship with the NHS means they can deliver specially tailored, bespoke care programmes supporting the physical and mental health of patients and staff.

Healthy Futures Social Prescribing Service, Wolseley Trust

Sarah Corbett and the Healthy Futures Social Prescribing Service, Wolseley Trust in Plymouth successfully supported hundreds of people, quickly developing a different approach to social prescribing with input from all team members. Social Prescribers have continued to support people to improve wellbeing by focussing on what matters to them. In addition, the team made over 1,500 welfare check calls to ensure people had all they needed to stay physically, mentally, and socially well during lockdown.

UPIC Health, Virginia, USA

Mary Tucker CEO, UPIC Health, Virginia, USA who when COVID-19 struck, immediately recognised the gap between the desperate need for care and availability of affordable and appropriate care. Mary stepped into the gap by quickly launching My Care Village, a free health coaching telehealth service. She hired certified health coaches, added recovery training for them, then trained them on Mend, a telehealth platform and NowPow, a personalized community referral platform.
