Updated: Secure Compass User ID Form
The Secure Compass User ID Form directors and supervisors fill out to create a new Compass staff account, change an existing staff account, or remove Compass access is updated and posted to the COWIC website.  It is now called the Compass Access Request Form and supervisors will need to acknowledge on the form that staff who no longer work for WIC have had their access removed to CIIS and wichealth.org. Links on how to do this are included on the form. Please note this form is now electronic and should be sent in by email to our Compass care team at COWICcompass@state.co.us.
