How have CKD programs and clinics adopted virtual care?
The Canadian CKD Clinic Network recently hosted two webinars highlighting adaptations made to kidney care due to the COVID-19 pandemic.
 
On November 19, Dr. Claudio Rigatto shared Manitoba's experience with virtual care, and Mary Beaucage and Susan McKenzie discussed how the Transplant Ambassador Program has pivoted to a virtual model that responds to patient needs in the wake of COVID-19. https://youtu.be/PgTjFKKHD9g

Bridging the Gap: Implementing the Transitional Care Approach for New Hemodialysis Patients

The study aims to improve the transition of new patients on HD to long term treatment of their chronic kidney condition by providing them a dedicated space for their treatment, facilitating ongoing support from the multidisciplinary team, and implementing an educational program tailored to their learning needs.
