Required Diocesan Lesson
Each year the diocese requires that all students in grades 1-8 enrolled in Religious Education complete a required lesson entitled Formation in Christian Chastity. Kindergarten students do not participate. Parents of high school students enrolled in faith formation will receive a separate email.
This required lesson is typically completed in the classroom with parents having the option to opt out and teach the lesson at home. Since this year we are not meeting in person for class and everyone is part of an at home program, parents will cover the topics at home.
We are including a link to a letter to parents for each grade level that highlights the topics to speak with your children about. Once you have covered the topics, you will submit the linked form below so that the Faith Formation Team can submit a report to Bishop Burbidge letting him know of student participation.

Please complete this form and submit it the faith formation team by Friday, January 22 so that we may submit a report to the diocese.

- You can put all of your children's name and grade levels on one form.
You will need to print and submit the form in one the following ways:
- Scan and send to pk12ff@gs-cc.org
- Drop off at the parish office - attention Stacy
- Mail to the parish office - attention Stacy
*If you are not able to print the form, we will have blank forms available for you to fill out and submit when you pick up your 3rd quarter materials (January 11-17).
We must submit a report to the diocese at the end of the month so it is important that you complete the form indicating that you have taught the lesson at home.
