We are now able to offer the COVID-19 vaccine to your extended family (siblings, in-laws, aunts, uncles, cousins)who areover the age of 65 and are Florida residents. The vaccine will be administered by The Florida Division of Emergency Management this Thursday, February 11, followed by the second dose on Thursday, March 11 at our Palmetto Bay Campus.
There is a limited amount of vaccines, so if you are interested, please complete this form with your family member's information by 5 pm today, Monday, February 8th and someone will contact them directly to provide more details. Please note that completing the form does not guarantee your family member will receive the vaccine.
Also, be reminded that this event is by invitation only and we ask you to abide by the following:
DO NOT share on social media about the vaccination site before, during, or after the event.
DO NOT invite or make promises on helping someone receive the vaccine.
