Optic reconstructive surgery in treatment of acute angle closure

Patients with acute angle closure often have paralytic mydriasis accompanied by neurotrophic changes of iris stroma. The decreased function of the iris diaphragm function leads to visual aberrations that compromise the vision related quality of life. In this video the authors describe a technique for reconstruction of the iris and mechanical opening of the anterior chamber angle. The surgical technique involves goniosynechiolysis to expose the trabecular meshwork and iridoplasty with a purse string 10-0 propylene to correct the mydriasis and also stretch the iris out preventing irido-trabecular contact.

Following a highly traumatic road traffic accident, this patient's iris was completely avulsed and remarkably cleanly expressed from the eye, leaving the IOL in the bag and relatively little collateral damage. The ciliary processes, Pan Retinal Photocoagulation scars and the equatorial region of the capsular bag are easily visible.
