COVID-19 Testing and Consent Form
The Governor's Cluster Action Initiative and the New York State Department of Health (NYSDOH) requires schools providing in-person instruction to test specific percentages of in-person students, teachers, and staff for COVID-19 if the school is in a designated yellow, orange, or red zone, in order to hold in-person teaching.
Our school is exploring the potential for school-based testing. With the planned start of high-risk sports we want to make every reasonable effort to support a healthy school community. If this program moves forward, your consent will ensure that your child is eligible to participate.

Athletics Updates
Several steps are required for a safe return to sports. The Heuvelton CSD High-Risk Sports Plan and supporting documentation are available on the school district website for review. The information below is provided to help families and student-athletes prepare.

Every parent and student-athlete must review and sign a COVID-19 Student Permission form.

School must also have a recent physical examination on record for each student-athlete. Physical exams may be completed by the family’s Primary Care Provider or by the school Medical Director & Nurse, using the Student Health History form. Physical exams will be offered at Heuvelton Central School on Thursday, February 11 between 11a.m.- 2:30 p.m, and on Friday by appointment.

If a student-athlete has previously tested positive for COVID-19, the COVID Return to Play form must also be completed.

Community Dinner
The Heuvelton Volunteer Fire & EMS Departments will be providing a FREE community dinner on February 27th.
Meal will include: ham, mashed potatoes, gravy, vegetable, roll, & dessert. Meals will be served at the fire department from 4:00-6:00 p.m. Please click here to sign up. These will be take out dinners only. Your response will help them estimate their food order.

Travel Advisory
With winter break coming up, our community should be aware of New York State travel restrictions. The travel restrictions were issued by the Governor in an executive order, which carries the force of law and must be adhered to.
Anyone traveling to a noncontiguous state will be subject to rules (set by New York State) which could impact a child’s ability to return to school when break is over.
We urge all families to familiarize themselves with these guidelines and rules prior to traveling.

Governor Andrew M. Cuomo has announced new guidelines allowing travelers to New York to “test out” of the mandatory 10-day quarantine for asymptomatic individuals. Travelers from states that are contiguous with New York are exempt from the travel advisory; however covered travelers must continue to fill out the Traveler Health Form. Essential workers will continue to be exempt as well. The guidance also applies to any person who has traveled to CDC Levels 2 and higher Health Notice country under the COVID risk assessment.
For any traveler to New York State from a noncontiguous state, US territory or CDC level 2 and higher country, the new guidelines for travelers to test-out of the mandatory 10-day quarantine are below:
For travelers who were out-of-state for more than 24 hours:
- Travelers must obtain a test within three days of departure, prior to arrival in New York.
- The traveler must, upon arrival in New York, quarantine for three days.
- On day 4 of their quarantine, the traveler must obtain another COVID test. If both tests comes back negative, the traveler may exit quarantine early upon receipt of the second negative diagnostic test.
For travelers who were out-of-state for less than 24 hours:
- The traveler does not need a test prior to their departure from the other state, and does not need to quarantine upon arrival in New York State.
- However, the traveler must fill out our traveler form upon entry into New York State, and take a COVID diagnostic test 4 days after their arrival in New York.
The local health department must make contact with the state the traveler came from, to ensure contact tracing proceeds there as well. All travelers must continue to fill out our traveler form upon arrival into New York State to contribute to New York State’s robust contact tracing program.
The travel guidelines require all New Yorkers, as well as those visiting from out-of-state, to take personal responsibility for compliance in the best interest of public health and safety.
