There have been many questions about the therapy treatment delivered in Skilled Nursing Facilities.  Especially those delivered by the overly aggressive contract therapy companies.  Therapists must have a more scientific basis for their practice.  Evidence based treatment is indeed best practice but contract therapy companies gave up on this years ago.  Their caseloads are usually determined by “Med B unit goals” or staff “productivity” expectations.  Standardized tests are on of the best ways to assist in determining a patient’s status on their initial therapy evaluation providing an objective assessment of the patient’s specific strengths and needs.  Standard tests are even mentioned in the Medicare Benefit Manual Chapter 15 regarding therapy services.  But as of now, they are only a suggestion and not a requirement.

#5. The Mini Mental.

The (Mini Mental Status Exam) MMSE is commonly used because it only takes 5 – 10 minutes and doctors or nurses don’t need any equipment or special training. The test has 30 questions that are each worth 1 point. These questions test memory, orientation, and math skills. 

If someone does score in the "cognitive impairment" range, it can indicate cognitive issues. But it doesn’t mean that they have dementia. What it does mean is that more physical and cognitive testing should be done to better understand what’s causing the cognitive issues.

#4. The Berg Balance Scale.

The Berg Balance Scale (BBS) is a valid, efficient measure of postural balance in the geriatric population.  The BBS is a qualitative measure that assesses balance via performing functional activities such as reaching, bending, transferring, and standing that incorporates most components of postural control.  BBS scores predicted the occurrence of multiple falls among the elderly and were strongly correlated with functional and motor performance in stroke patients.  Although the BBS is widely used and can distinguish fallers from non-fallers, it lacks a gait assessment component.

#3. Tinetti Assessment Tool.

 The Tinetti Assessment Tool is a simple, easily administered test that measures a resident’s gait and balance. The test is scored on the resident’s ability to perform specific tasks. It is appropriate for adults and the elderly. Residents who score in the range of 19 – 24 points indicate that the resident has a risk for falls.

Interrater reliability was measured in a study of 15 residents by having a physician and a nurse test the residents at the same time. Agreement was found on over 85% of the time and the items that differed never did so by more than 10%. These results indicate that the Tinetti Assessment Tool has good interrater reliability.

#2. Elderly Mobility Scale.

The Elderly Mobility Scale (EMS) is a 20 point validated assessment tool for the assessment of frail elderly residents. EMS is an ordinal scale measurement.  EMS evaluates an individual’s mobility problems through seven functional activities including bed mobility, transfers and bodily reaction to perturbation. Speed of sit to stand and walking speed are analyzed.  You do need a stop watch, measuring tape and access to a bed and chair to complete this test. This is an older test that many have forgotten about, but it is still a good one.

#1. Barthel Index of Activities of Daily Living. 

The Barthel index is an ordinal scale that measures functional independence in the domains of personal care and mobility in patients with chronic, disabling conditions, especially in the rehabilitation settings. Two main versions exist: the original 10-item form and expanded 15-item version. The 10-item version is the most used; it includes evaluation of independency in feeding, moving from wheelchair to bed and return, grooming, transferring to and from a toilet, bathing, walking on a level surface, going up and down stairs, dressing, and continence of bowels and bladder.  And as a proud OTR, you  know this had to be number 1.
