COVID Vaccination Clinic

The Guild has been working with a COVID Vaccination Clinic with the intent on having a COVID vaccine readily available to Brewery Members & their staff. The vaccine is subject to availability and there are a lot of logistics to negotiate so we here asking for your help.
Details
- Date: Monday, April 5th 2021 (backup date Monday, April 12th 2021) We need the backup date incase of delay of vaccine doses. You will need to be available both days prior to signing up.
- Location: PHX Beer Co. Brewery + Taproom (3002 E Washington St, Phoenix, AZ 85034).
- It will be the Johnson & Johnson vaccine, which is a single dose.
Requirements
- You must be an employee/employer of one of the Arizona Craft Brewers Guild Brewery Members.
- We ask everyone to bring insurance cards but that’s just for the clinic's reimbursement, everyone gets the shot for free regardless of insurance.
What I Need to Know Now
I need to gather an accurate number of doses that we will need for the day. Please respond by email with what brewery you are with and how many will be getting vaccinated in your company.
