This series consists of four sessions

Dates: Thursday, April 29, 2021
Time: 6:00pm - 8:00pm
Location: Virtual Zoom Meeting (details to follow)
Who Should Attend: Family Doctor or Nurse Practitioner, MOA, RN in practice, Office Manager, Community Health Nurse (CHN), MHSU Clinician or other allied health support that you work with on a day-to-day basis.
Please note: If you did not attend session 1, you are still welcome to attend this session.

Foundations of Team Based Care

Topics:
- Determine what team based care means in your practice & the value it brings to you and your patients
- Define roles and responsibilities within your team & determine how everyone can work at the top of their scope

Session 3:
Inter-Professional Communication
Thursday, June 3, 2021
6-8pm
|
|
|
Topics:
- Determine how your team will communicate considering differences in styles
- Reflect on your own communication style and how this may influence the team based care environment
- Determine strategies to prevent communication challenges and interpersonal conflict it causes
|

|
Session 2:
Team Care Philosophy

Topics:
- Determine what your team's primary care philosophy is and how it will benefit your panel of patients
- Define each team member role and accountabilities in your team care philosophy
- How do you communicate team roles and responsibilities to your patients?

Session 4:
Team Organization
Thursday, July 15, 2021
6-8pm
|
|
|
Topics:
- Create your team charter including purpose, mission and values
- Define and develop collaborative team strategies to use as a team for your panel of patients
- Identify and develop how you will measure the success of how your team is functioning
|
|
|
|
Sessional compensation and CEUs for these learning sessions are possible through the implementation of a quality improvement project in your clinic.
