Tdap Vaccine Requirements

Wisconsin law requires that children entering 6th grade either receive a Tdap vaccine or have a signed waiver.  A Tdap fact sheet is attached.  If your child will be in 6th grade next year, you may want to make your appointment now so that you are ready for school in August.  Each year we have families who have difficulty getting an appointment.   Please let the office know when your child has been vaccinated so immunization records may be updated.

Health Screenings

Please continue to do daily health screenings of your child to assist in maintaining in-person instruction. Health Screening.  It is imperative for you to daily monitor your child's health and keep them home should they exhibit symptoms.  Please use this additional checklist below to assist should your child have COVID-related symptoms. We have been blessed to maintain in-person learning. I pray that we can continue for the foreseeable future. Checklist
