If you are an adult with ADHD (age 18 and older), CHADD―the leading nonprofit organization providing support, training, education,

and advocacy for the ADHD community―wants to hear from you!
We invite you to take a few minutes (approximately three to five minutes) to respond to our Spring 2021 Survey, developed to help us learn more about the impact of the COVID-19 pandemic on the ADHD community.
In appreciation for your help, everyone who completes the survey will be entered into a random drawing for great prizes! A total of five winners will be selected.
