Family Survey

A Message from Eagle County Public Health and Environment
In the very near future, it is expected that youth aged 12-15 years will be eligible for vaccinations against COVID. In anticipation, Eagle County Public Health and Environment and Vail Health are planning clinics for youth in this age group.
More clinic info will be available once confirmed, but preliminary info on where vaccines will be offered are listed below for your planning purposes:

Wednesday in Vail at Vail Health

Wednesday in Gypsum at Eagle Valley High School

Thursday in El Jebel at the Eagle County Community Center

Saturday in Edwards at Battle Mountain High School
These clinics cannot happen until final approval is provided by the FDA and CDC. We don’t know the date this will happen, but these clinics will be established as quickly as possible following that approval.
Once the clinic schedule is confirmed, appointments can be made at www.eaglecountycovid.org. This can help secure a location and time of your preference, but appointments are not necessary. Each clinic will be open to walk-ins, while the Vail clinic will be walk-ins only. By hosting these four clinics, we hope to be able to provide a vaccination to everyone who wants one. Second dose clinics will then be scheduled three weeks later at the same locations and times. Additional walk-in clinics will also be available in Vail, Avon, and Eagle later in May and in June for anyone who cannot make these clinics.
We are providing this information now to help you begin to plan and understand what to expect. An announcement will go out confirming the dates and times and this information will be updated at www.eaglecountycovid.org.
Office Reminders
EXCUSED ABSENCE FORM- Please make sure to contact the office by 9am to notify your student absent. If you know in advance your child will be absent, please submit an (
Excused Absence Form) to the office. In addition, you should contact your child’s teacher to discuss any missed assignments.
CITY MARKET CARES - A program for nonprofits where a percent of sales is donated to your organization. Stone Creek Charter School is proudly participating in this FREE community rewards program. Each December, the participants in each nonprofit are removed from the database. Please take a couple of minutes to assign Stone Creek Charter School, organization number IW790 as your choice to benefit from this for 2020.
