"A 66-year-old musician, Mr. C. was a model methadone-clinic patient. After adhering to daily observed methadone treatment for years, he had ultimately been granted the privilege of 'take-home dosing,' so he had to go only every other week to pick up his methadone. For decades, methadone treatment had improved his quality of life and function, allowing him to work as a peer recovery coach and a musician at his church. That is, until his cancer diagnosis threatened to compromise his hard-earned recovery."

Imagine you or someone you love had to choose between treatment for opioid use disorder or cancer, because doing both was logistically impossible. This situation and potential reforms are explored in a piece published November 25 in the New England Journal of Medicine by authors including CHAMPP postdoctoral researcher Dr. Katie Fitzgerald Jones, CHAMPP Affiliate Faculty Dr. Paul Joudrey, and CHAMPP director Dr. Jessica Merlin. The case of Mr. C illustrates the impact that the proposed Modernizing Opioid Treatment Act, or MOTA, could have on people suffering from opioid addiction. The bill seeks to expand who can prescribe methadone and where it can be dispensed, eliminating the need for daily visits to a methadone clinic.

The Modernizing Opioid Treatment Act (MOTA)

The federal Modernizing Opioid Treatment Act, or MOTA, would expand methadone access to an additional 14% of census tracts in the U.S., according to new research from the University of Pittsburgh published recently in Health Affairs Scholar.

The proposed law would change not only who can prescribe methadone, but also where it can be dispensed. Currently, only clinicians at federally certified opioid treatment programs can prescribe and dispense the drug for opioid use disorder, which studies have shown is one of the most effective treatments for the condition. Often, a patient must visit a clinic each day to receive their dose. Under MOTA, any addiction medicine physician or psychiatrist could prescribe the drug, and it would be available at pharmacies in addition to certified clinics. Because of the limited number of addiction specialist physicians in the U.S., researchers examined if broader prescribing privileges, including prescribing by primary care clinicians, may be necessary to meet the scale of the overdose epidemic.
