1095-C Forms

Your 1095-C forms are available now through FileCloud in the ACA Forms folder. According to federal law, it is your responsibility to provide these forms to your employees no later than March 1, 2024.
  • It is recommended you provide a paper copy of the form to eligible employees.
  • Employees (with ongoing employment since 2022) can also log into bswift via www.ascendtowholeness.org (click on My Services, then Benefits Management) and access their 1095-C forms on the home page under “My Forms.”
If you do not see your 1095-C forms in FileCloud, there are outstanding questions and issues that need to be addressed. Please check your inbox for an email from benefits@adventistrisk.org
1094-C Forms:
If you only offer the Ascend to Wholeness Healthcare Plan to your employees, your 1094-C form (along with the respective 1095-C forms) will be submitted electronically to the IRS by April 1, 2024.

If you offer other health plans to your employees and completed the “1095-C Acknowledgement Form” as required, ARM will create the 1095-C forms based on the information in bswift and provide the forms to you. It is your responsibility to file the 1094-C form along with these documents to the IRS.
 

Process Reminders

As we are starting the new year, here are a few process reminders to ensure your employees’ demographic information is correct, along with helpful reports in bswift to support you in validating your data entry and/or making updates.

Do’s:
  • Regular timely maintenance of the records within 30 days of change in bswift
    • New Hires, Employment Changes, and Terminations
    • Salary/Compensation information — ACA compliance and Long-term Disability benefit billing
    • Update address changes and contact information for log in support
  • Double check and verify records
  • Maintain eligible dependent records — e.g. SSN, overage, and ineligible.
  • Make sure records match with legal documents — i.e. no nicknames.
  • Utilize “Re-Hire” for employees who have a termination date and have a break in employment
Don’ts:
  • Do not add prefixes in the “Suffix” field — e.g. Mr., Mrs., DMin., Pastor., etc.
  • Do not fill/change the Shared Responsibility fields on the Employment page
    • We will complete this section based on key data fields you maintain
  • Do not add a “Re-Hire” and termination date when there is a Time Status change – e.g. employee going from part-time to full-time.
  • Do not add non-benefit eligible employees – i.e. seasonal and student workers (per recent guidance)
Helpful Reports and Processes in bswift:
  • Salary Import Template — if making a significant number of updates (20 records or more)
  • Evidence of Insurability (EOI) — to check for pending EOI
  • Overage Dependents — dependents turning 26 years old
  • Benefit Elections Report — summary of active elections
  • International hire with no SSN as of hire date

Effective January 1, 2024, $100,000 Employer-paid basic life insurance is now matched in employer-paid accidental death and dismemberment (AD&D) benefit (unless the employee opted into the $50K basic life which is matched by $50K in AD&D). As approved by the NAD Risk Management Committee, this upgrade in employee benefits applies to all full-time employees receiving the employer-paid basic life benefit.
  • Voya Premium Templates: Updated with new rates for the AD&D benefit (added to the basic life rate). 2024 excel template is available in FileCloud in your main folder. Please download a template copy to your computer.
  • 2024 Basic Employer AD&D rate is $.013 per $1,000 in volume. E.g., for $100,000 the rate is $1.30 PEPM.

Question: I’m confused on what effective dates I should use when I’m doing a Life Event in bswift. Is there a Life Event breakdown?

Answer: Life Events can be a bit confusing! Here is a table that can guide you on what effective dates you should use.

 
Life Event Effective Date
Marital Status Day of the event – e.g. date of marriage or divorce
Dependents Day of event – e.g. date of birth or adoption
Gain of Employee/Spouse/Dependent Benefit Eligibility Elsewhere Day after the last full day they should have coverage – e.g. Leaving January 31, effective date is February 1
Loss of Employee/Spouse/Dependent Benefit Eligibility Elsewhere Day after benefits ended – e.g. lost benefits on June 31, effective date is July 1
Death Day after the event – e.g. Passed away on February 2, effective date February 3
Employment Status Change Day of status change – part-time to full-time
