Once you have completed the sign up form, email diabetes@ky.gov to let us know what areas you need to be tagged on (example:DPP, DPP coaches, KDPCP Monthly Notes, HLWD etc.)
Introducing Diabetes Kickstart, a newly released series of animated videos created by the CDC and designed to raise awareness of and access to DSMES services. They are based on the ADCES7 Self-Care Behaviors™ framework and end with a call to action to seek a referral for DSMES.
New training available on TRAIN Kentucky is designed to help the CHW understand the basics of nutrition and how they can implement their knowledge in their clients’ lives. The following topics will be discussed; Effects of carbohydrates on blood sugar, Foods containing carbohydrates, and Information on the new nutrition labels. It will also discuss; Choosing healthy foods can help manage diabetes and managing diabetes can help delay or prevent complications. CHWs who complete the module will earn 1CHW CEU.
