Santa Clara Family Health Plan (SCFHP) partners with community agencies to provide your patients with health education classes and resources for their wellbeing.
All classes and programs are provided at no cost to SCFHP members.
These classes include:
- Chronic Disease Self-Management
- Exercise & Fitness
- Nutrition & Weight Management
- Prenatal Education
- At Risk for Type II Diabetes
- Women’s Health

Please remember that provider contracts are being updated for a new dual eligible special needs plan called Santa Clara Family Health Plan DualConnect (HMO D-SNP). – and we encourage you to complete and return your contract as soon as possible. If you did not receive a contract, or have questions please email us at contracting@scfhp.com.
Enrollment for new members is currently underway and will have an effective date of January 1, 2023. Our 10,300+ Cal MediConnect plan members will transition to DualConnect on January 1, 2023. There will be no gap in the transitioning members’ Medi-Cal, Medicare and Medicare Part D prescription drug benefits.
DualConnect will coordinate both Medicare and Medi-Cal services under one umbrella, making health care management easier for members – with one primary care doctor, one membership card, and one phone number to call for help.
Please return your completed contracting agreement to SCFHP right away.

SCFHP Cal MediConnect Plan is changing to SCFHP DualConnect (HMO D-SNP) on January 1, 2023.
Members may have recently received a notice in error that says the plan is ending. Members should be advised to ignore that termination notice and that no action is needed to continue receiving their health coverage from SCFHP. The notice is labeled H7890_15111E in the bottom left corner.
Cal MediConnect Plan members will be automatically enrolled in SCFHP DualConnect and will remain with their current primary care providers who are contracted with SCFHP DualConnect.
We appreciate your assistance in addressing any questions from your patients about these notices. We apologize for any confusion our error may have caused.
