Interprofessional Electives

IPEC 563: Interprofessional Complex Care Coordination

Pre-requisites: None
Credit Hours: 2 or 3 credit options
Tuesdays 4:00PM - 5:00PM
Weekly 1-hour seminars with other self-scheduled field experiences at the VCU Health Hub at 25th.

With the interprofessional student team and faculty support, students will engage with community members to provide care transition and coordination support including goal development and connecting patient(s) to community resources as applicable. In teams, students will present a final case study of your patient(s) to include assessment, problem identification, interventions, ability to link patients to resources, outcomes, lessons learned, and implications for future professional practice. In-class meetings provide relevant content for working with patients with complex needs and time to work with an interprofessional student team. Class content topics include trauma-informed care, social determinants of health, motivational interviewing and highlighting various community resources available to patients in Richmond and surrounding areas.
