Thank you for participating in the CVH Area Network calls to discuss, learn, and support your colleagues this past year! Over the year, we hosted 24 calls over three quarters with participation from almost every state. Topics discussed during the September and June calls included the following:
  • State Plans
  • Community Pharmacy Partnerships
  • Medication Therapy Management
  • Sustainability Planning, Implementation and Tools
  • Z Codes
  • Team-Based Care
  • Adoption and Utilization of EMRs
  • Self-Measured Blood Pressure
Recordings of the calls can be found here.

We will not be hosting calls in December or January, but we will be available to respond to any requests. Please access the recordings and let us know if you need any resources or help connecting with your colleagues. We are happy to support you!

Calls and Webinars Worth Your Time

National Association of Community Health Workers (NACHW) Wants Your Input: Join a Feedback Session Dec. 8, 2022, 2-3:30 p.m. ET

Sponsor: NACHW

NACHW is preparing to expand materials and resources available within the Document Resource Center (DRC), the nation’s largest searchable collection of documents on policies around community health workers (CHWs). This database is free to access on the NACHW website. Anyone seeking guidance and practical tools to support CHW policy development can use it.

During this one-time feedback session, you will be asked for feedback on key terms used in the CHW field. Be a part of the solution and strengthen the DRC search feature and the resources available to our national community.
Objectives: By the end of the feedback session, participants will assist NACHW to:
  • Develop various meanings of the terms “evidence base” and “employment practices.”
  • Identify problems and potential benefits of these terms.
  • Identify potential solutions to broaden the search for resource documents to add to the DRC.
Register here for this one-time only feedback session.

News and Resources from CDC, NACDD, and National Partners

From CDC Division for Heart Disease and Stroke Prevention, Applied Research and Evaluation Branch: NEW RESOURCE! Best Practices for Heart Disease and Stroke: A Guide to Effective Approaches and Strategies

Cardiovascular disease continues to be leading causes of death each year. Despite efforts to mitigate impacts of heart disease and stroke through programs and policies, system-based barriers still impede progress toward reducing complications, deaths, and costs associated with heart disease and stroke.

To prioritize effective and sustainable strategies that are critical to implementing heart disease and stroke prevention and management intervention, the Division for Heart Disease and Stroke Prevention developed the Best Practices for Heart Disease and Stroke: A Guide to Effective Approaches and Strategies.

The updated Best Practices Guide includes 18 evidence-based strategies, highlighting the public health, economic, and health equity impacts of each strategy. In addition, it provides information and resources to support and guide strategy implementation and policy and law-related considerations. Brief stories highlight specific settings where these strategies have been successfully implemented.

You can learn more about this Guide on Feb. 14, 2023, from 2:30-3 p.m. ET on the Coffee Break webinar: The Best Practices Guide for Heart Disease and Stroke: Exploring What’s NewClick here to save the date directly to your calendar.

Centers for Disease Control and Prevention (CDC) Division for Heart Disease and Stroke Prevention (DHDSP) call for nominations

CDC DHDSP, in conjunction with NORC at the University of Chicago (NORC), is seeking nominations of health care organizations that are currently implementing anti-racism initiatives, practices, interventions, policies, or activities with the potential to reduce health disparities and advance health equity. Nominated sites will have the opportunity to participate in an evaluation of the impact of their anti-racism practices on health care delivery, health equity, and patient-level outcomes related to heart disease, stroke, and other chronic disease conditions.

We hope you will consider nominating a clinic or health system affiliated with your organization or your partnering organization. Please review the announcement here and flyer here for more information.

The deadline to submit nominations is Friday, December 9, 2022.
