Detailed below are two types of surveys that can be used with MailChimp - one is a survey asking for opinions, etc and the other can be used to gather and tabulate votes on various issues. Please follow the instructions for both that are detailed below. I will share with you what the backend reports look like at our upcoming meeting so please complete by our Friday meeting.

SURVEY EXAMPLE:
Per the conversation we had about using MailChimp to create and send surveys to the state members, the link to the survey we created in MailChimp that was sent to the PDX members can be accessed below. This survey used a wide variety of question types; free form text, multiple choice, mandatory & non-mandatory, etc.
** Click on the Take Survey button
**. Answer the questions - don't worry about how you answer them.
Some of the responses are mandatory and are identified by an * As part of the test, skip some of the responses with an asterisk and once you submit you should receive an error message before you are not allowed to submit without completing those mandatory questions. Go back to that question, check a response and then submit - you should then be able to submit it. Responses that are mandatory are 1, 3, 4, 5, 6, 8
** Once you have submitted your response, go back and try to take the survey again. You should receive a message that you have already submitted it and not be allowed to do it again.
** The survey is anonymous unless we capture an email address within the survey. Our survey asked people to submit their email address as an optional field - surprisingly, most of the PDX members included their email address which linked to their first/last names in the report that was produced by MailChimp. If people choose not to add their email address, they are anonymous to us.
** Once you have completed the survey, I will capture the report created by MailChimp and share that with you.
