Regarding the 2023 registration forms, thank you to those that have already sent them in. To those that have not, this is a friendly reminder to please send in your membership form and check.
Please complete your membership application (click on the link below) and return it together with your check. Checks should be payable to: Connecticut Cemetery Association Inc. Please mail your application and check to: Anthony Puglisi, Treasurer, CCA, c/o Evergreen Cemetery, 769 Ella T. Grasso Boulevard, New Haven, CT 06519 on or before January 15, 2023.
The membership form is fillable. So please feel free to fill it in, save it and then email it to Anthony Puglisi at arpuglisi@evergreencem.org. You can mail your check separately. Please be sure to fill out the registration form completely. We use the information you provide for hard mailings, emails and future membership directories.
