It's Time to Update Your Roster

Please send the following information for all current employees,
attached in an Excel spreadsheet:

• Employee First and Last Name
• Employee Social Security Number
• Employee Classification
  • e.g. Non‑DOT/FMCSA/PHMSA

Additionally, for all current employees classified as FMCSA:

• CDL Driver's License Number and Issuing State
  • Note: the first two characters of the CDL Driver’s License Number must be a valid U.S. state or Canadian province code, or MX for Mexico. Please do not include any spaces or hyphens.
