Part of care coordination includes health promotion. That's why your Care Manager is involved in helping you coordinate services to promote optimal health. Connecting with a primary care physician and annual physicals and dental exams are essential to preventative care. Care Managers can assist you with finding and obtaining providers and services in specialty areas. 

You can also search the Community Resources Tool to find a physician or dentist in your community.
