Dear Downtown East Toronto members, colleagues and community,

Welcome to the June 2023 issue of the DET OHT's newsletter. In here, you will find key highlights, including one of the on-going projects of the DET OHT, spotlight on a valued community member, and news from the Ministry of Health and City of Toronto.

On a regular basis, the newsletter will highlight key initiatives and programs underway and inspiring stories of people who make DET OHT's mission a reality, including its team and community members. You are welcome to share this communication with your extended networks.

Current Projects
Digital Shared Care System
In a step towards creating a more seamless care experience for patients/clients in the downtown core, the Downtown East Toronto Ontario Health team (DET OHT) launched a pilot project that focuses on improving care pathways between organizations for people experiencing precarious housing and facing health accessibility challenges.

The aim of the ‘Digital Shared Care System’ pilot project is to digitally connect organizations to share patient/client information in a safe and timely manner that adheres to all privacy legislation to address gaps in care, improve efficiency and provide a better care experience for patients/clients. To read more, click here.
What is New
The New and Official DET OHT Logo

We are thrilled to reveal the new and official logo of the DET OHT, a symbol of an inclusive and integrated system of care and collaborative growth. 

In December 2022, we embarked on an exciting quest to establish a unique visual identity and brand, working closely with diverse stakeholders both within and beyond the DET OHT. Over the course of seven months, we conducted extensive research, held insightful discussions, and gathered valuable feedback through engagement surveys. This exciting process enabled us to create something that represents the DET OHT accurately. 

While our logo has a new and fresh look, our unwavering vision remains unchanged: to provide our community with inclusive, inspired and responsive care through one connected system of diverse providers who enthusiastically embody holistic approaches to improving population and individual health and wellbeing.

 
Toronto Health and Social Services Directory
The Toronto Health and Social Services Directory is now live. It is a website that helps people more easily find health, social, and community services in Toronto, including the former municipalities of East York, Etobicoke, North York, Scarborough, and York. 

Features of the Toronto Health and Social Services Directory include:
  • Ability to search 8,000+ services by location combined with topic or keyword
  • Easy-to-find service topics, including Older Adults, Caregivers, Mental Health, Health Care, 2SLGBTQI+, Newcomers, Income Support, Housing and more
  • Ability to search for services offered in specific languages
  • Detailed service listings that include eligibility, address, hours, accessibility and contact information
This directory is an initiative of the eight Ontario Health Teams serving Toronto and serves to complement existing navigation resources in Ontario.

PRIDE 2023

The eight Ontario Health Teams of Toronto walked together in Pride Parade on July 25, 2023 to support collaborative and inclusive delivery of care in the capital of the province. Approximately 100 people, including members of the Toronto OHTs and their families and friends, registered for the march. 

Toronto OHTs also volunteered at the information booth for awareness around health, community, and social care services, including the 2SLGBTQIA+ services, for people living and/or receiving care in Toronto. The booth operated from Friday, June 23 through Sunday, June 25 and served an opportunity for the colleagues at Toronto OHTs to engage with their communities. 
Hear From Our Community Members
A Conversation with Veronica Snooks
We invited Veronica Snooks, a member of the DET OHT Community Advisory Council (CAC) and the Mental Health and Addictions (MHA) Working Group, for an engaging and insightful conversation. Veronica lends her expert knowledge and perspectives to the CAC and MHA Working Group as a person with lived experience and a member of the First Nation with the goal of enhancing the delivery of care for patients and clients in the Downtown East area. As she reflected on her life journey, she spoke about her motivation to join the DET OHT, most memorable contribution, and hopes from the DET OHT as it matures over time.
