Start with information for athlete #1 (A1), then tab down for athlete #2 (A2). If you don't have additional athletes, leave A3 and A4 blank.

Add parent/guardian #1(PG1) information then add parent/guardian #2 (PG2) - note there are fields for PG3 and PG4 if you want to include additional parents/guardians
